What visceral fat actually is
Not all belly fat is the same. Subcutaneous fat sits just under the skin — it is the fat you can pinch. Visceral fat sits deeper, wrapped around the liver, pancreas and intestines. It is firm rather than soft, which is why a stubborn midsection can feel tight rather than doughy, and why waist circumference tells you more than the scale.
Visceral fat is metabolically active tissue. It releases inflammatory signals and free fatty acids directly into the portal vein feeding the liver, which drives insulin resistance, fatty liver, higher triglycerides and higher blood pressure. This is why it is associated with cardiovascular and metabolic disease far more strongly than overall body weight.
It also behaves differently under stress. Chronically elevated cortisol preferentially stores fat in the abdomen, so people under sustained stress or sleeping poorly often gain visceral fat without a large change in weight. In women, the shift in estrogen through perimenopause moves fat storage from hips toward the abdomen for the same reason.
The encouraging part: visceral fat is more metabolically responsive than subcutaneous fat. When someone begins losing weight, visceral fat is typically the first to shrink — meaning early improvements in labs and waist measurement often arrive before the mirror shows much.
How these protocols target it
Tirzepatide is the anchor. Its dual GIP/GLP-1 action lowers appetite, steadies blood sugar and improves insulin sensitivity — and imaging studies of GLP-1 and dual-agonist therapy consistently show visceral and liver fat falling proportionally more than total body fat. Lower circulating insulin also removes one of the main signals telling abdominal fat cells to store rather than release.
MOTS-c is a mitochondrial-derived peptide studied for how it influences metabolic flexibility — how readily the body switches to burning fat for fuel. In animal work it improved insulin sensitivity and reduced diet-induced fat gain, and it is used clinically alongside training rather than as a substitute for it.
Sermorelin stimulates your own pituitary to release growth hormone in natural pulses. Growth hormone is lipolytic and has a documented preference for abdominal fat — one of the clearest findings in adult GH-deficiency research is a fall in visceral fat. Sermorelin also improves deep sleep, and improved sleep lowers the cortisol pattern that drives abdominal storage.
NAD+ supports the cellular machinery all of this depends on: mitochondrial energy production and the enzymes that regulate fat metabolism. It is a supporting player, most useful for the fatigue that makes training and food discipline hard to sustain.

No peptide is FDA-approved specifically for reducing visceral fat. Tirzepatide and semaglutide are approved for chronic weight management and type 2 diabetes, and the visceral- and liver-fat reductions seen in imaging substudies are secondary findings within that approved use. Tesamorelin is the only agent with an FDA approval tied directly to visceral fat, and only for HIV-associated lipodystrophy — it is not part of our formulary. MOTS-c evidence in humans is early; most of what we know comes from animal and cell studies. Sermorelin's abdominal-fat effect is inferred from growth-hormone physiology and adult GH-deficiency research rather than trials in healthy adults with central obesity. We prescribe with that hierarchy of evidence stated plainly.
How visceral fat protocol is dosed
Protocols start with what your labs show. Fasting insulin, HbA1c, a full lipid panel, liver enzymes and — where relevant — thyroid and morning testosterone or estradiol tell us whether we are treating insulin resistance, a hormonal shift, or a fatty liver that needs its own attention.
Tirzepatide is prescribed as either a standard titration for meaningful total fat loss, or a microdose protocol for clients whose weight is close to where they want it but whose waist and metabolic markers are not. Microdosing tends to be the better fit for lean-but-central body types and for people who train seriously and cannot afford appetite suppression that undercuts fuel intake.
MOTS-c and sermorelin are typically layered in after the first month, once tolerance to tirzepatide is established. Sermorelin is dosed at night to align with natural growth-hormone pulses. NAD+ is added when fatigue is a limiting factor.
Expect waist measurement, not weight, to be the primary marker. We measure at the navel every two weeks. Lab improvements usually appear at the eight- to twelve-week recheck; visible change in the midsection generally takes three to six months. Protein at every meal and two to three resistance sessions a week are not optional additions — without them a meaningful share of the loss comes from muscle.

Safety, side effects, and who should avoid it
A large waist with normal weight can also reflect conditions that peptides do not treat — Cushing's syndrome, significant alcohol-related liver disease, ascites, or an abdominal wall separation after pregnancy. Part of the intake is ruling those out rather than assuming the answer is a fat-loss protocol.

- • GLP-1 and dual-agonist medications are not appropriate with a personal or family history of medullary thyroid carcinoma or MEN2, or with a history of pancreatitis.

- • Heavy alcohol intake will blunt results and continues to drive liver fat — we address it directly rather than around it.
- • Peptides do not tighten loose abdominal skin and do not repair diastasis recti; those need surgical or physical-therapy answers.
- • Spot reduction is not real. These protocols shift where the body stores and releases fat; they do not target a body part.

Can I lose visceral fat without medication?

Do peptides target belly fat specifically?
No peptide burns fat in one location on demand. What tirzepatide and sermorelin do is shift the metabolic and hormonal conditions — lower insulin, natural growth-hormone pulses — under which the body is most willing to release abdominal fat. In imaging studies that shows up as visceral fat falling faster than total fat, but it is a systemic effect, not spot reduction.
How fast does visceral fat come down?
Faster than most people expect in the labs and slower than they hope in the mirror. Liver enzymes, triglycerides and fasting glucose often improve within eight to twelve weeks. Visible change in the midsection typically takes three to six months of consistent adherence.
Should I use microdose or standard tirzepatide?
