What glutathione is, and why the liver depends on it
Glutathione is a small protein built from three amino acids — glutamate, cysteine, and glycine — that your cells make themselves. It is the body's primary intracellular antioxidant, and the liver is both its largest production site and its largest consumer. Nearly every compound your liver processes, from medications and alcohol to environmental chemicals and the byproducts of your own metabolism, passes through a pathway that spends glutathione.
That spending is the point. In what biochemists call phase II detoxification, enzymes attach glutathione to reactive molecules produced by phase I metabolism, converting them into water-soluble compounds the body can excrete through bile and urine. Without adequate glutathione, those reactive intermediates linger — and the oxidative stress they create is a well-established driver of liver cell injury.
The clearest proof of this relationship is in emergency medicine: the standard hospital treatment for acetaminophen overdose is intravenous N-acetylcysteine, given specifically to restore depleted liver glutathione. That is not a wellness theory — it is established, life-saving practice, and it tells you how central glutathione is to hepatic defense.
How MTHFR variants connect to glutathione
The MTHFR gene codes for an enzyme in the methylation cycle. It converts folate into 5-MTHF, the active form required to recycle homocysteine back into methionine. The common C677T variant can reduce that enzyme's activity substantially — roughly 30 to 65 percent in people who carry two copies — and the A1298C variant has a milder effect. The practical result, particularly when folate or B12 status is low, is often elevated homocysteine.
Homocysteine sits at a metabolic fork. It can be recycled back to methionine, or it can be sent down the transsulfuration pathway, where a B6-dependent enzyme converts it toward cysteine — the rate-limiting building block for making glutathione. This is the link people with MTHFR variants are usually asking about: when methylation is impaired and cofactor status is marginal, both arms of that fork can run inefficiently, which may reduce the raw material available for glutathione synthesis.
We want to be straightforward about the strength of this link. The biochemistry is well characterized and the mechanism is real. What is not established by large clinical trials is that a specific MTHFR genotype reliably produces measurably lower glutathione levels, or that injected glutathione is the correct answer for it. In many cases, optimizing methylfolate, B12, and B6 — and using precursors like NAC and glycine — addresses the pathway more directly. That is a conversation to have with a clinician who has seen your labs.

Clinical trial evidence for injectable glutathione as a liver therapy is preliminary: the published studies are small, short, and often open-label without a placebo group. No trial has shown that glutathione reverses fatty liver or fibrosis. The MTHFR-to-glutathione connection is mechanistically sound but has not been validated by outcome trials.
How glutathione is dosed
Oral glutathione is largely broken down by enzymes in the gut before it reaches circulation, which is why pharmacokinetic studies have repeatedly failed to show meaningful increases in blood levels from capsules. Injectable delivery bypasses gut degradation, and it is the only form we prescribe.
There is no single validated dose for liver support. Published research protocols have used doses in the range of several hundred milligrams, and clinical practice varies widely by patient and pharmacy. Safety, side effects, and who should avoid it
Glutathione is generally well tolerated in short-term use. The most commonly reported effects are mild injection-site redness or tenderness, and occasionally transient flushing or mild nausea. Allergic-type reactions are uncommon but possible.

Will glutathione injections fix my fatty liver?
No, and we will not tell you otherwise. Small preliminary studies have shown modest improvements in liver enzymes with injectable glutathione, but no trial has demonstrated that it reverses fatty liver or fibrosis. Reducing alcohol, improving diet, losing excess weight, and managing blood sugar remain the proven foundation. Glutathione is used to support antioxidant capacity alongside that work.
Why can't I just take a glutathione pill?
Oral glutathione is broken down by enzymes in the gut before much of it can be absorbed, and pharmacokinetic studies have shown little to no meaningful rise in blood levels from oral dosing. That is why physician-supervised protocols use injectable delivery, which bypasses digestion entirely.
Do I need glutathione because I have an MTHFR variant?
Not necessarily. MTHFR variants impair methylation and, in theory, can reduce the cysteine available for glutathione synthesis — but this has not been proven to require glutathione specifically. Many clinicians address methylfolate, B12, and B6 status first, since those act directly on the affected pathway. A physician review of your labs, including homocysteine, is the right starting point.
Is injectable glutathione FDA-approved?

Can glutathione replace NAC or milk thistle?
They are complementary rather than interchangeable. NAC is the better-studied precursor and is the FDA-approved treatment for restoring liver glutathione in acetaminophen toxicity. Glycine is another building block. Milk thistle has its own modest evidence base. Your clinician may combine glutathione with precursors and cofactors rather than using it alone.
Will glutathione help me detox from alcohol faster?
There is no strong clinical evidence that glutathione injections accelerate alcohol clearance. What is well established is that chronic alcohol intake depletes hepatic glutathione and increases oxidative stress, which is the rationale for repletion in people reducing their drinking. If you drink heavily and daily, talk to a physician before changing your intake — abrupt withdrawal can be medically dangerous.
Should I get MTHFR testing before starting?
It is reasonable to discuss, particularly if you have unexplained elevated homocysteine or a family history. Routine MTHFR testing is not recommended for the general population by major guidelines, and a variant on its own — with normal homocysteine and no clinical findings — usually does not change management.
Is injected glutathione the same as what my liver makes?
Chemically, yes — it is the identical molecule. But getting glutathione into the bloodstream is not the same as your hepatocytes synthesizing and using it internally, and how much reaches the inside of liver cells at useful concentrations is not fully characterized. This is one of the honest open questions in the field.
